    This form is required to obtain a Fire Operational Permit to conduct a carnival, fair, or other temporary public event within the City of Woodland or the Springlake Fire Protection District. A permit ensures that fire and life safety requirements are met for events that involve temporary structures, tents or canopies, vendors, cooking operations, generators, amusement activities, or large gatherings of people. Event organizers must complete and submit this form at least 30 days before the event to allow for plan review and scheduling of any required inspections. Additional documentation such as site plans, tent layouts, and vendor lists may be required. Submission of this form does not guarantee permit approval. A member of the Community Risk Reduction Division will contact you with next steps and fee information after your application is received.

Provide a brief summary of your event, including the main activities planned, type of attractions (e.g., food vendors, live music, rides, games), and any special features such as cooking operations, generators, or open flame use. This helps the fire department understand the scope of your event and determine what safety requirements may apply.

    • Locations of tents, canopies, booths, rides, stages, cooking areas

    • Distance Between Tents/Structures

    • Fire lanes, access roads, hydrants

    • Exits and assembly areas

    • Location of Generators or Electrical Equipment

    • Location of Propane Cylinders or Fuel Storage (if any)

    • Fire Extinguisher Locations

  17. Will fireworks, pyrotechnics, or open flame features be used?*
  18. Will there be amusement rides or inflatables?*
  19. List vendor and state permit # if applicable
